APPLICATION FOR ADMISSIONS

Please read the instructions carefully when completing the Application for Admission. Type all the required information and make sure
that all mandatory items have been answered. Payment of an application fee of 195 euros is required. The school will only process the
Application for Admission once it has received ALL of the application requirements. Although we may initially receive scanned copies of
the supporting documents, please note that we also must receive all the originals by post or by courier within days of the admission
confirmation. Original official school transcript of the most recent three years must be received directly from the schools.

Completed Application for Admission (must be signed by a parent or guardian)
Completed Attendance Commitment.
Completed Waiver of Liability.
Completed Access to Spanish Universities Form or Homologation of 4 ESO Form
Completed Use of Images Authorization Form
Certified copy or original transcripts 1.) in the original language and 2.) with an official certified English translation, if not already in
English. Original transcripts become property of ES American School and will not be returned.
Application fee of €195 *(non-refundable).
Copy of student’s passport and parents’ passports
Copy of student’s and parents’ DNI card (for Spanish nationals) or NIE card (for non-Spanish nationals with a residency number).
Passport size photograph
Signature of BOTH parents is required in ALL forms
Signature or stamp of recruitment agency on the Application for Admission, if applicable.
Send the completed application and supporting documents listed above directly to ES American School.
